In some cases, detection of a serological immune response (acute and convalescent antibody levels) to extracellular products of Streptococcus pyogenes (Strep Group A) can be helpful in the diagnosis of the poststreptococcal sequelae acute glomerulonephritis (PSGN) and acute rheumatic fever (ARF). The two classic such streptococcal products are streptolysin O (a cellular hemolysin) and DNase B. Anti-DNAse B antibody levels remain detectable longer than do antistreptolysin O (ASO) levels and are generally more reliable in documenting a previous S. pyogenes infection. Commonly, both anti-ASO and anti-DNase B levels are performed at the same time to decrease the chance of false-negative results.
